New Patient Forms

All patient must complete new forms. Please complete the registration form, HIPPA release, appropriate questionaire, and records request is you have not already done so. Note the Privacy Policy is below for your information only. You do not need to print it.

Records can be obtained by completing this form, and dropping it off at Madison Behavioral and Diagnostic Treatment Services.  

Records should be obtained within 30 days of the request being received.  Answers to questions about your medical records can be found HERE.

New patient forms can be downloaded using this page, or you can arrive early and fill them out prior to your visit.
