Previous patients may request their records be sent to the Slaughter Rd office.  A written request is required for your previous records. We request you ask for your records prior to your first appointment but will see patients without them.  We can request this information once you see Dr. Swartz and sign our form.

Madison Behavioral and Diagnostic Treamtent services is located at 1643 Slaughter Road. The office is located south of Hwy 72 and north of Eastview Drive.
